Monitor clients' health status and report any changes or concerns to healthcare professionals or family members promptly. Support clients with meal preparation and nutritional needs tailored to their dietary requirements. Help clients stay engaged through recreational activities, hobbies, or outings as appropriate. Follow care plans meticulously, ensuring all services are delivered safely and respectfully. Experience Prior experience working as an in-home caregiver or in a similar personal care role is preferred. Experience supporting clients with geriatrics or post-acute care needs is highly valued. Knowledge of medical-surgical procedures, outpatient and clinic settings enhances your ability to deliver comprehensive care. Inpatient and sub-acute care experience supports delivering consistent, high-quality support in various settings. Certification or training related to geriatrics, post-acute care, or outpatient services is advantageous for providing specialized assistance.
