Job Description

Overview We are excited to hire a dynamic and compassionate Personal Care Life Skills Coach to join our team in Fallston location. This vital role involves empowering individuals in the Independence Program to lead a self-directed life to the greatest extent. As a Life Skills Coach, you will provide personalized support, facilitate skill-building activities, improve problem-solving skills, contribute to home life, enhance community engagement, and promote positive behavioral health outcomes. This paid position offers an opportunity to make a meaningful difference in the lives of those you serve while working in a supportive and energetic environment. Responsibilities Develop and implement individualized plans focused on enhancing daily living skills, social skills, and behavioral management for clients. Assist with
IADLs:
meal planning, errands and handling household management tasks. Support clients in community settings such as attending volunteer work, education and work, and ensuring safe transportation and engagement in social activities and opportunities. Help Client stay motivated with fitness and wellness goals. Collect and document data related to client progress, behavior patterns, and skill acquisition to inform ongoing care strategies. Utilize effective communication skills to foster understanding and connection with individuals. Break tasks into simple steps and provide follow-through. Collaborate with LSC Supervisor and NRM Case Manager to coordinate care plans and ensure consistent support. Requirements Prior experience working with individuals with behavioral health needs or in human services, coaching, or senior support. Knowledge of behavior management techniques. Experience in home & community care environments is highly desirable. Strong communication skills and capable of data collection, maintaining detailed reccords of client progress and incidents. Ability to assist with IADLs and provide coaching support compassionately. A clean background check
Availability:
Tuesdays and Thursdays from 10a-3p weekly First Aid and CPR Certified Reliable transportation with a valid Driver's License and clean driving record
Pay:
$18.00 per hour
