Job Description
Weekend Community Support & Personal Care Assistant Bay City, MI Full-Time | Weekend-Focused Position We are seeking a dependable and compassionate individual to provide one-on-one support for an adult with physical and developmental disabilities through a Self-Determination Community Living Supports (CLS) program. This is one client in one private home—not a facility and not multiple clients. Guaranteed 40-Hour Schedule Saturday and Sunday, 5:00 a.m. to 9:00 p.m. This position guarantees 40 hours per week. Saturday and Sunday are the primary required workdays, with scheduled weekend shifts from 5:00 a.m. to 9:00 p.m. Applicants must be available to work both days on a regular basis. The remaining weekly hours will be scheduled during the week based on the individual's needs, staffing coverage, school schedule, and available shifts. Weekday hours may include daytime, evening, overnight, school-break, or summer coverage. Additional open shifts may also become available for pickup. What You'll Be Doing This is a hands-on position focused on helping the individual participate in daily life, work toward Person-Centered Plan (PCP) goals, and remain active at home and in the community.
Responsibilities include:
Supporting Person-Centered Plan (PCP) goals Assisting with community outings and activities Encouraging independence and participation in daily living activities Providing hands-on personal care, hygiene, toileting, and brief changes Performing Hoyer lift transfers Assisting with mobility and positioning Assisting with standing programs using adaptive equipment Supporting the individual in working toward use of a standard gait trainer Assisting with meals and self-feeding Providing personal care and mobility assistance during community outings Completing required documentation Maintaining a safe, clean, and respectful environment Physical Requirements This is a physically active caregiving position. Duties include lifting and assisting with Hoyer transfers, bending, stooping, kneeling, repositioning, hands-on personal care, and pushing or maneuvering adaptive mobility equipment. Applicants must be able to safely perform the essential functions of the position, with or without reasonable accommodation. Qualifications Must be consistently available Saturday and Sunday from 5:00 a.m. to 9:00 p.m. Able to work a guaranteed 40-hour weekly schedule Able to work the required weekend shifts and scheduled weekday hours Dependable attendance Reliable transportation Comfortable providing hands-on personal care, including toileting and brief changes Comfortable with Hoyer lift transfers and mobility assistance Able to safely perform the physical requirements of the position Able to follow established care instructions and complete required documentation Previous experience as a Caregiver, DSP, CLS Worker, CNA, Home Health Aide, Personal Care Assistant, or similar role is preferred but not required. Paid training is provided. This position is best suited for someone who wants 40 guaranteed hours per week, can commit to working Saturday and Sunday from 5:00 a.m. to 9:00 p.m., and is available to work the remaining scheduled hours during the week.Pay:
