This role involves working closely with residents in group homes, assisted living facilities, or home and community care settings. The ideal candidate will be committed to promoting independence, safety, and well-being while adhering to all relevant healthcare standards and regulations. Prior experience in long-term care, working with individuals with developmental disabilities is required. This position offers an opportunity to make a meaningful difference in the lives of those you serve. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor residents' health status by taking vital signs and observing for any changes in condition Administer medications following care plans and HIPAA guidelines Support residents during meal preparation and feeding, ensuring nutritional needs are met Implement individualized care plans tailored to each resident's needs and preferences Provide emotional support and companionship to promote mental well-being Maintain a clean and safe environment through regular cleaning and sanitation tasks Use specialized equipment such as Hoyer lifts safely and effectively for resident transfers Document care activities accurately and report any concerns to supervisors or healthcare professionals Assist with transportation or outings as needed, ensuring safety at all times Must have weekend availability Skills Certification in First Aid and CPR required; knowledge of medication administration required Must have DWC training completed and can do live Recipient Rights Training. Experience working in group homes, assisted living, nursing homes, or home health environments is advantageous Strong observational skills for patient monitoring and recognizing changes in condition Ability to assist with ADLs, including personal hygiene, mobility, and meal preparation Familiarity with dementia care, Alzheimer's care, memory care, autism support, and hospice care is beneficial Knowledge of HIPAA regulations to ensure resident confidentiality and privacy Excellent communication skills for effective interaction with residents, families, and team members Ability to perform heavy lifting safely when necessary; experience with resident transfer equipment such as Hoyer lifts is a plus Compassionate attitude with a focus on person-centered care and social work principles for supporting individuals with disabilities or long-term conditions This role provides an enriching opportunity to contribute positively to the lives of individuals requiring ongoing support.
