Job Description
General Summary This position performs a variety of direct service functions for persons served who are enrolled in day program services, including services for adults with developmental disabilities and adults with serious mental illnesses. Essential Functions Directly supervises consumers during program activities (both on site and during community outings). Carries out daily program plans as prescribed through the individual plan of service of the person served and as assigned by the supervisor. Provides transportation services when necessary. Assists with personal care needs of persons served Assists persons served with the administration of medication as prescribed. Assists with the preparation of daily programming, organizing and assembling materials, and proper cleanup and storage of materials. Participates in person centered planning-related meetings as requested. Documents the progress of persons served through the completion of clinical service notes. Completes other agency documentation as required.
