Our mission is to provide our clients with reliable and respectful health care in a home-like setting and provide opportunities for our employees' well-being and development. Job Overview At Lifestone, our small assisted living setting allows team members to get to know residents personally and provide individualized, hands-on care. We are looking for dependable and caring staff who enjoy working with older adults, value teamwork, and want to make a meaningful difference in residents' daily lives. Responsibilities Provide personal care assistance, including help with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques. Monitor residents' health by observing vital signs, tracking changes, and reporting concerns promptly to healthcare professionals. Assist with medication administration following prescribed plans while adhering to HIPAA regulations and resident rights. Prepare nutritious meals tailored to individual dietary needs, including special diets such as dementia or Alzheimer's care. Maintain a clean, safe, and comfortable environment by performing light cleaning duties and ensuring infection control protocols are followed. Support residents with mobility using equipment such as Hoyer lifts or other assistive devices as needed. Facilitate engaging activities that promote mental stimulation and social interaction, especially for individuals with neurodevelopmental or developmental disabilities. Document care activities accurately in care plans and communicate effectively with team members regarding resident progress. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Knowledge of patient observation techniques, medication administration, and vital signs monitoring. Familiarity with dementia care and resident's rights regulations. Ability to perform heavy lifting and assist with patient transfers safely using manual handling techniques or specialized equipment such as Hoyer lifts. Certification in CPR and First Aid is preferred; ongoing training in hospice or end-of-life care is a plus. Strong communication skills for effective client interaction and team collaboration. Ability to handle multiple responsibilities efficiently while maintaining a compassionate approach.
