Overview Join our dedicated team as a Hybrid Role In Home Direct Support Caregiver and Coordinator, where you will play a vital role in providing compassionate, personalized care to individuals with diverse needs. This dynamic position combines hands-on caregiving with coordination responsibilities, ensuring clients receive comprehensive support in their homes and community settings. You'll be empowered to make a meaningful difference by assisting with daily living activities, monitoring health and safety, and coordinating care plans—all while fostering independence and dignity. Responsibilities Deliver direct support to clients by assisting with activities of daily living (ADLs), including personal care, meal preparation, medication reminders, and companionship. Observe and monitor patients' health status, and behavioral changes; document findings accurately in compliance with HIPAA regulations. Implement individualized care plans tailored to each client's needs, including dementia care, Alzheimer's care, memory care, autism support, and neurodevelopmental or intellectual disabilities. Coordinate services by communicating effectively with clients, families, healthcare providers, and community resources to ensure seamless care delivery. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and sanitation in accordance with health standards. Assist with transportation needs for appointments or community outings while ensuring client safety during transfers and travel. Support clients' rights by respecting their preferences and promoting independence within the scope of caregiving responsibilities. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong patient observation skills to identify changes in condition or behavior promptly. Knowledge of patient monitoring techniques Familiarity with HIPAA regulations to ensure confidentiality of patient information. Experience supporting individuals with dementia, Alzheimer's disease, autism spectrum disorder (ASD), developmental disabilities, or neurodevelopmental disorders. Excellent communication skills for effective client interaction and team collaboration. Competency in emergency response procedures such as CPR and first aid certification. Ability to adapt caregiving techniques to meet the unique needs of each individual while maintaining professionalism and compassion.
