Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote comfort and independence. Monitor patients' health by observing vital signs, reporting changes, and maintaining accurate care plans in compliance with HIPAA regulations. Support medication administration and ensure adherence to prescribed treatment schedules under supervision or as trained. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Facilitate patient transfer methods using equipment such as Hoyer lifts and manual handling techniques to ensure safety during mobility. Provide companionship by engaging clients in meaningful activities, conversations, and recreational pursuits to enhance emotional well-being. Maintain a clean and safe environment through regular cleaning duties, laundry assistance, and organization of personal spaces. Support clients with transportation needs for medical appointments, errands, or community outings while ensuring their safety. Follow all care plans diligently, including specialized dementia care, Alzheimer's care, memory care, or hospice support as required. Document patient observations accurately and communicate effectively with healthcare teams regarding patient status or concerns. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, vital signs monitoring, medication administration, and HIPAA compliance. Demonstrated ability to assist with ADLs and perform manual handling tasks safely; experience with Hoyer lifts and patient transfer methods is advantageous. Certifications such as CPR and First Aid are required; additional training in dementia care or neurodevelopmental disorders is a plus. Strong communication skills with the ability to connect empathetically with clients of diverse backgrounds including those with autism or developmental disabilities. Ability to lift heavy objects safely and perform physical tasks associated with caregiving roles consistently.
