Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patients' health status by observing overall condition, reporting any changes promptly Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking Utilize specialized equipment such as Hoyer lifts or other assistive devices to ensure safe patient transfers and mobility Maintain detailed documentation of care provided, patient observations, and any incidents in accordance with healthcare standards Requirements Proven experience in home & community care or assisted living environments; group home experience is a plus Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or wheelchairs when necessary Strong understanding of dementia care, hospice care, autism support, and working with individuals with developmental disabilities or disabilities in general Excellent communication skills to collaborate effectively with clients' families and healthcare teams Valid driver's license and reliable transportation preferred for travel between client homes Join us in delivering compassionate home health services that truly make a difference!
