Job Overview We are seeking a compassionate, energetic, and dedicated AFL (Assisted Living and Family Care) Provider to join our dynamic team. Responsibilities Provide direct personal care assistance, including helping residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility transfers using equipment like Hoyer lifts and patient transfer methods. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes promptly to healthcare teams. Assist with medication administration following HIPAA guidelines and care plans to ensure safety and compliance. Support residents with meal preparation, feeding assistance, and maintaining proper nutrition tailored to individual dietary needs. Conduct patient observation for signs of dementia, Alzheimer's disease, or other neurodevelopmental disorders; implement individualized care plans accordingly. Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control protocols. Facilitate transportation for appointments or community outings while ensuring resident safety during transfers and personal transportation assistance. Skills Proven experience working in group homes, nursing homes, assisted living facilities, or home health environments. Strong knowledge of patient monitoring techniques, medication administration, and vital signs measurement. Familiarity with dementia care, Alzheimer's care, memory care strategies, and resident's rights advocacy. Ability to assist with ADLs using manual handling techniques and equipment such as Hoyer lifts; capable of heavy lifting when necessary. Excellent communication skills for effective client interaction and team collaboration; understanding of HIPAA privacy standards. Experience working with individuals with autism or intellectual disabilities; supporting those with developmental disabilities or neurodevelopmental disorders. Knowledge of emergency procedures including CPR and first aid certification; ability to respond swiftly in urgent situations. Competence in meal preparation, personal transportation assistance, and home & community care practices. Demonstrated ability to follow detailed care plans while providing compassionate caregiving tailored to each resident's needs.
