Associate/Qualified Professional Schedule:

Monday - Friday, 8:00 am to 5:00 pm with weekends on call for emergencies

Pay Rate:

$21.00 per hour

Position Duties:

Assist with hiring qualified direct care staff to work with each individual consumer, including the verification that all required documentation (background check, NC Healthcare Registry check, employment verifications, etc.) have been completed prior to worker beginning direct care hours with consumer. Assist with coordinating all required training for each Habilitation Technician prior to the staff working with the consumer(s). Also responsibility for ensuring that annual refresher trainings are completed in a timely manner. Provide CPR/FA and Safety Care training to clinical and administrative staff upon request. Assist with verifying accuracy of service authorizations received from PIHP. Follow-up in writing for any approvals not received within required guidelines. Write short term goals for each Innovations waiver consumer and complete quarterly progress assessment for each consumer (as required by PIHP.) For consumers receiving state-funded services, develop the PCP and complete the SNAP assessment prior to starting services and on an annual basis thereafter. Assist with ensuring backup staff is available and works all shifts when primary direct care staff is not available. Complete Level 1 incident report required for all missed shifts Lead by exemplifying company standards: including promotion of consumers rights and respect for consumer and family at all times and a safe and productive work environment. Assist with reviewing of all documentation completed by Habilitation Technicians to ensure accuracy, completeness and adherence to all Medicaid standards. Ensure documentation is completed in timely manner and forwarded to billing and payroll When a direct care staff violates agency policy, provide disciplinary action following agency protocol and document same. Fill in for direct care as needed. On call responsibilities as needed for consumer and employee emergencies.

Qualifications:

Minimum Education:

Bachelors Degree in a related health & human services field At least 2 years of experience working with the IDD population
