This vital role involves providing comprehensive in-home support to promote independence, safety, and well-being for clients across various stages of life and health conditions. Whether assisting with daily activities or managing complex medical needs, you will be a trusted caregiver committed to delivering exceptional patient-centered care in a home environment. This paid position offers opportunities to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Duties Provide direct patient care by assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately Administer medications following prescribed schedules and medical documentation protocols while ensuring compliance with HIPAA regulations Support patients with specialized needs such as catheter care, wound management, Alzheimer's and dementia care, and hospice support Operate medical equipment like Hoyer lifts and assist with transfers safely to prevent injury Maintain detailed medical records using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation of patient progress Assist with household tasks including meal preparation, cleaning, laundry, and maintaining a safe living environment to promote overall well-being Collaborate with healthcare teams to ensure continuity of care and communicate any changes in patient condition promptly Requirements Valid certification as a Home Care Aide or similar credential recognized in the industry Proven experience in home health care, assisted living, nursing homes, or long-term care settings Strong knowledge of medical terminology and procedures related to patient observation and medication administration Hands-on experience working with individuals with developmental disabilities or memory-related conditions such as Alzheimer's disease or dementia Ability to operate EMR/EHR systems efficiently for accurate medical documentation CPR certification and knowledge of emergency response procedures including hospice care protocols Competency in using assistive devices like Hoyer lifts and performing vital signs assessments accurately Excellent caregiving skills combined with compassion, patience, and respect for client privacy under HIPAA guidelines Flexibility to work live-in shifts or flexible hours based on client needs Join us in providing compassionate home health services that empower clients to live comfortably and safely in their own homes.
