Key Responsibilities:

Assist with daily living activities such as personal hygiene, meal preparation, and mobility Follow Individualized Care Plans (ICPs) tailored to each client's needs Provide companionship, emotional support, and social engagement Assist with medication reminders and monitor overall well-being Support clients with transportation to appointments and community outings Maintain accurate care documentation and follow care guidelines.

Qualifications:

High School Diploma or GED At least 18 years of age Prior home health aid experience preferred Valid Driver's License and reliable transportation CPR and First Aid certification HHA license-preferred Why Join SYNERGY HomeCare?
