Receive and process referrals from various sources, such as hospitals, physicians, and other healthcare providers. Collaborate with internal teams to determine patient eligibility and initiate the intake process. Serve as the primary point of contact for patients, families, and healthcare professionals regarding care coordination. Address inquiries, concerns, and provide guidance on accessing services and resources. Schedule patient appointments, coordinating availability of healthcare professionals and patient preferences. Monitor and update schedules to accommodate changes, cancelations, and rescheduling needs. Communicate pertinent patient information, including diagnosis, treatment plans, and care instructions, to the appropriate individuals. Maintain accurate and up-to-date electronic and physical records of patient information, appointments, and communication. Provide compassionate and empathetic support to patients and their families throughout the care coordination process.
