Description Overview Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver to provide delegated personal care services to children and adolescents in a home-based setting in the Albuquerque, NM area . In this role, you will support clients with daily living activities, supervision, and routine care, ensuring comfort, safety, and dignity in the home environment. This flexible, on-call position is ideal for a reliable caregiver who is patient, observant, and passionate about supporting youth and families. Key Responsibilities Provide in-home supervision and support for children and adolescents Assist with personal care tasks including bathing, dressing, grooming, toileting, and eating Follow individualized care plans and delegated care instructions from licensed professionals Support medication reminders as delegated by care plan and clinical team Provide mobility assistance and support with safe positioning as needed Maintain a clean, safe, and structured home environment Offer emotional support, companionship, and age-appropriate engagement Support daily routines, behavioral structure, and consistency in care Escort or transport clients to appointments or activities as needed Observe, document, and report changes in behavior, mood, or condition to the clinical team What We're Looking For Compassionate, patient, and dependable caregivers Experience working with children or adolescents preferred Strong communication and interpersonal skills Ability to follow care plans and work independently in a home setting Calm, attentive, and safety-focused mindset Ability to maintain structure and routine in a home environment Reliable and consistent PRN availability Qualifications Must be at least 18 years of age Previous caregiving, childcare, or youth support experience preferred Ability to follow instructions accurately and maintain structured care routines Strong observational and communication skills Valid driver's license and reliable transportation preferred Must live in Albuquerque, NM or surrounding areas Why Join Us At Heritage Home Healthcare, we are committed to providing compassionate, personalized in-home support that helps children, adolescents, and families thrive.
