Description Overview Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver in the West Albuquerque, NM area (87121) to provide high-quality in-home care to clients. In this role, you will provide delegated personal care services in a home setting, supporting clients with activities of daily living while ensuring comfort, safety, and dignity. This flexible, on-call position is ideal for a reliable caregiver who is patient, observant, and committed to providing consistent, client-centered care. Coverage Area West Albuquerque, NM - ZIP Code 87121 Key Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and eating Follow individualized care plans and delegated care instructions provided by licensed professionals Provide mobility assistance, including transfers, repositioning, and wheelchair support Support medication reminders and delegated care tasks per care plan and agency guidelines Maintain a clean, safe, and organized home environment through light housekeeping assistance Provide companionship, emotional support, and encouragement to clients Escort or transport clients to appointments or errands as needed Observe, document, and report changes in client condition, behavior, or care needs to the clinical team Maintain professionalism, confidentiality, and compassionate client-centered care at all times What We're Looking For Compassionate, dependable, and patient caregivers Strong communication and time-management skills Ability to work independently in a home setting Strong attention to detail and observational skills Commitment to providing high-quality in-home care Willingness to travel between client homes in the Albuquerque area Flexible availability for PRN scheduling needs Qualifications Must be at least 18 years of age High school diploma or GED preferred, but not required Previous caregiving experience preferred Ability to follow instructions and care plans accurately Strong interpersonal and communication skills Must have a current valid driver's license Reliable transportation and valid auto insurance required Must live in Albuquerque, NM or surrounding areas Schedule PRN / as-needed scheduling Flexible availability required Schedule may include weekdays, weekends, evenings, or holidays based on client needs Benefits Competitive wages Flexible PRN scheduling opportunities Ongoing training and professional development Supportive and collaborative work environment Why Join Us?
