About Us:
At Sloan Comfort Care Home (SCCH), a program of Accountable Health Community, our mission is to provide deeply compassionate end-of-life care that respects the dignity and autonomy of each guest. By caring for only two guests at a time, we create a peaceful, home-like setting where individuals and their families receive personalized attention, comfort, and support during this profound stage of life.
Position Summary:
We are seeking compassionate and dedicated Caregivers to join our team at SCCH. As a Caregiver, you will play a critical role in delivering quality end-of-life care and support to our guests, their loved ones, and visitors. You will be responsible for ensuring the smooth operation of the home while embodying the values and philosophy of Sloan Comfort Care Home. We are especially seeking candidates who have overnight availability.
General Duties:
Quality Care:
Provide exceptional end-of-life care and support to guests, their loved ones, and visitors, maintaining high standards of dignity and compassion at all times through respecting autonomy and choice.
Household Support:
Assist in various tasks as needed to ensure the efficient functioning of the home and household.
Mission Alignment:
Demonstrate attitudes and actions that align with the mission and philosophy of Sloan Comfort Care Home.
Functional Duties:
Care Plan Implementation:
Read and follow the personal care interventions outlined in each guest's care plan.
Daily Schedule Adherence:
Follow daily schedule guidelines, communicate effectively regarding guest conditions, and maintain time and attendance standards.
Routine Duties:
Perform routine personal care and household tasks, including cleaning, laundry, cooking, and phone communication.
Team Collaboration:
Work collaboratively with volunteers and other staff members.
Medication Management:
Handle medications safely according to SCCH training and other policies, ensuring accurate charting of medications and treatments in real-time.
Issue Reporting:
Notify supervisor of any issues, including safety concerns, and inform of any changes in guest condition with detailed information.
Requirements:
Orientation and Training:
Complete paid SCCH house orientation and caregiver checklist.
Background Checks:
Ability to successfully pass criminal background and drug testing.
Communication Skills:
Demonstrate effective communication skills and the ability to interact compassionately with guests, families, and staff.
Mission Fit:
Exhibit actions and attitudes consistent with SCCH's mission and end-of-life caregiving principles.
Physical Ability:
Capable of lifting up to 35 pounds and handling potential exposure to body fluids and airborne contaminants, using universal protective equipment as required.
