Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly to healthcare providers Support medication administration following proper protocols while maintaining accurate medical documentation in Electronic Health Record (EHR) systems Provide companionship and emotional support to clients living with dementia or Alzheimer's disease, ensuring a safe and comforting environment Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a clean and safe living space Assist with medical procedures like catheter care and hospice-related tasks under supervision or guidance from nursing staff Maintain strict compliance with HIPAA regulations to protect patient privacy and confidentiality at all times Qualifications Proven experience in home health care, assisted living, nursing homes or long-term care environments Strong knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation Certification in CPR is required; additional certifications such as medication administration or hospice care are a plus Experience working with individuals with developmental disabilities or dementia enhances your ability to provide specialized care Ability to perform patient observation accurately and respond effectively to changing needs or emergencies Valid driver's license and reliable transportation preferred for live-in or community-based assignments Compassionate attitude combined with excellent communication skills to build trust with clients and families Join us in delivering compassionate home health services that empower individuals to live safely and comfortably in their own homes.
