DESCRIPTION

Coordinates referrals from Nyack Hospital and all other referral sources (i.e., Westchester, New York City and New Jersey Hospitals, Rehab and Sub Acute Facilities, physician's offices, etc.), to develop appropriate home care plans together with the physician, referring organization, patient, and family. Conducts and participates in Quality Assurance Performance Improvement activities appropriate to the Home Health Agency.

QUALIFICATIONS

BSN, or Bachelor/Masters/Doctor of Science Degree in Physical Therapy, or Bachelor Bachelor/Masters/Doctor of Science Degree in Occupational Therapy or Master of Social Work Degree. Two years' clinical experience and two-year Home Care experience. Current license to practice as a registered professional nurse, physical therapist, occupational therapist or social worker in New York State. Valid/Current Drivers License required, basic knowledge of MS Excel is a plus $55,000 STARTING AT
