Job Description

Overview Join our dedicated team as a CDPAP Caregiver seeking Clients, where your compassion and expertise will make a meaningful difference in the lives of individuals requiring personalized home care. This role offers an exciting opportunity to provide vital support to clients with diverse needs, including seniors, individuals with disabilities, and those managing chronic conditions. As a caregiver, you will deliver exceptional patient-centered care in a respectful, empowering environment that values your skills and commitment. Whether you're experienced in assisted living, hospice care, or working with individuals with developmental disabilities, your efforts will help clients maintain independence and improve their quality of life. Duties Provide compassionate assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient observations, including vital signs and overall health status, ensuring safety at all times Assist with medication administration and manage medical documentation accurately using Electronic Health Record (EHR) systems Support clients with specialized needs such as catheter care, Alzheimer's and dementia care, and hospice support Operate medical equipment like Hoyer lifts and perform safe transfers to prevent injury Prepare nutritious meals and assist with light housekeeping tasks including cleaning and laundry to maintain a safe living environment Document all patient interactions thoroughly while adhering to HIPAA regulations to ensure confidentiality and privacy Qualifications Proven experience in home health, long-term care, or assisted living environments; nursing home experience is highly valued Knowledge of medical terminology, medication administration, and vital signs monitoring Certification in CPR and first aid required; additional certifications such as hospice care or dementia training are a plus Ability to work independently while managing multiple client needs effectively Strong communication skills for working with clients who have developmental disabilities or memory impairments Familiarity with EMR/EHR systems for accurate medical documentation Valid driver's license and reliable transportation preferred for client visits in the community Join us in delivering exceptional care that transforms lives!
