Home Care PRN Location:
Greater Dayton Area Schedule:

Key Responsibilities Provide direct personal care to hospice patients in home settings Assist with activities of daily living (ADLs) including bathing, grooming, oral care, dressing, and toileting Support patient comfort and safety , including proper use of medical equipment (oxygen, bed rails, etc.) Perform skin care and positioning to prevent breakdown and promote comfort Assist with meal preparation, feeding, and light household tasks as assigned Take and document vital signs (temperature, pulse, respirations, blood pressure) as directed Observe and report changes in patient condition to the RN Case Manager Provide emotional support and companionship to patients and families Offer respite care for caregivers when needed Maintain accurate, timely documentation in compliance with hospice standards Required Qualifications Minimum 16 years of age Active State Tested Nursing Assistant (STNA) / Certified Nursing Assistant (CNA) certification Listed on the state nurse aide registry in good standing Current BLS Passionate care to terminally ill patients Strong communication and observational skills Preferred Qualifications Prior experience in hospice, home health, or long-term care Experience supporting patients with chronic or terminal conditions Benefits & Perks Supportive, team-centered culture Medical, Dental, and Vision Insurance (effective first of the month) 401(k) with company match Generous Paid Time Off (PTO) and paid holidays Mileage reimbursement Tuition reimbursement & career growth opportunities Employee referral bonus program Merit-based pay increases Employee discount programs Why Join Day City Hospice?
