  • Full-time Job Summary We are seeking a dedicated and experienced Case Manager for Senior In-Home Care to manage comprehensive care plans tailored to the needs of elderly clients receiving in-home support.
The ideal candidate will possess a strong background in healthcare case management and has a LPN or equivalent license, with expertise in managing complex cases involving acute, chronic, and palliative conditions. The position helps with reassessing and updating care needs and must able to know how to handle senior care. As a Case Manager, you will play a vital role in facilitating seamless patient care transitions, coordinating services and scheduling, and ensuring compliance with company regulations. Your expertise in management will contribute significantly to high-quality care and communication in our senior home care environment. You will serve as a vital link between clients, families, healthcare providers, and community resources to ensure optimal quality of life and safety for seniors receiving home-based care. Responsibilities Conduct thorough comprehensive assessments of clients' medical, psychological, and social needs to develop personalized individualized care plans. Coordinate services across multiple disciplines including family, nursing, social work, therapy, and hospice care to ensure seamless delivery and integration of client care. Monitor client progress regularly and adjust care plans accordingly to meet evolving health requirements. Support client, family and employee and follow-up care within hospital admission and re-admission. Facilitate discharge planning from hospitals or healthcare facilities by ensuring appropriate home care arrangements are in place. Educate clients and their families on health management strategies. Maintain and oversee detailed documentation, care plans, and review processes within electronic health record (EHR) systems while adhering to HIPAA regulations. Manage smooth transitions with scheduling and assigning employee to the case based on the client's needs and employee's experiences and availabilities. Monitor patient progress through evaluation and ongoing assessment to adjust care plans as necessary. Professional delicacy/etiquette phone answering, texting, and emailing. Recruit and hire direct care staff. Marketing company service. Perform office duties and weekend on call rotation. Able to perform client care if in a pinch. Requirements Proven experience in critical care, hospital settings, or emergency medicine with a focus on senior patient populations. Strong background in nursing, social work, or related healthcare fields; nursing licensure preferred. Background in senior in-home care, Dementia, Alzheimer, and Parkinson diseases. Familiarity with EMR (Electronic Medical Records) and EHR systems along with HIPAA compliance requirements. Familiarity with care in-take and re-evaluation, and case management practices. Ability to communicate complex medical terminology clearly to clients and families while demonstrating cultural sensitivity. Strong organizational skills with the capacity to manage multiple cases simultaneously in a fast-paced environment. Knowledge of managed care processes.
