- , you'll contribute to our success in the following ways: + Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery + Develop, review, and revise individualized plans of care based on patient needs + Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards are met + Monitor and report significant changes in patients' physical and mental conditions + Educate caregivers, patients, and families using effective teaching and learning principles + Ensure all services meet State, Federal, contract, and licensure requirements + Conduct caregiver orientations and performance assessments as required + Evaluate home environments for safety risks and recommend solutions to improve patient safety + Maintain accurate clinical documentation, care plans, and service records + Communicate professionally with caregivers, clients, case managers, and leadership teams + Participate in staff meetings, in-services, and continuing education opportunities to support professional growth + Support Elara Caring's mission, values, and commitment to exceptional patient-centered care What is Required?
