Job Description

In this vital role, you will provide comprehensive support to clients in their homes, fostering independence, safety, and comfort. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity. Monitor patients' health status by observing vital signs, medication adherence, and overall condition; report any concerns promptly. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support clients with mobility and transfers using proper techniques such as Hoyer lifts or patient transfer methods to ensure safety. Maintain detailed care plans and document all care activities, observations, and changes in patient condition in compliance with HIPAA regulations. Provide companionship and emotional support, engaging clients in meaningful activities to enhance mental well-being. Assist with household tasks such as cleaning, laundry, and light housekeeping to maintain a safe and comfortable living environment. Qualifications Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly preferred. Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), hospice care, and home health practices. Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using equipment like Hoyer lifts. Certification in CPR and First Aid is required; additional certifications Strong communication skills for effective client interaction and documentation; experience working with clients with autism or neurodevelopmental disorders is advantageous. Familiarity with medication administration, vital signs monitoring, care plans development, and assisted living procedures. Valid driver's license and reliable transportation for personal transportation assistance if needed.
