Full-time, Part-time and On-Call Available! Job Summary:
We are looking for a Direct Care Worker to join our team! In this role, you will make a difference in the lives of seniors and people with disabilities by enhancing their quality of life. You will work directly with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but also building genuine relationships with those you serve. The position requires a caregiver who is detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to be successful in this role. Prior Home Health or Hospice experience is a plus. The ideal candidate is patient, compassionate, and reliable. Direct Care Worker responsibilities include overseeing medication consumption, performing housekeeping duties, assisting patients with cooking meals, and taking care of personal hygiene needs throughout the day. Responsibilities:
Aid patients in daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
Perform home management functions such as light housekeeping duties, changing bed linens, doing laundry, and cleaning
Run errands, including shopping for groceries and essential household items
Assist patients with bathroom visits, personal hygiene and grooming needs, and dressing
Accompany patients with scheduled appointments
Remind patient of medications and immunizations as prescribed by healthcare providers
Plan and prepare meals and snacks, and assist clients with eating/drinking and clean-up
Ensure patient safety and maintain a safe environment.
Engage in mobility, walking, physical therapy exercises, and self-help activities with patients
Monitor the physical, mental, and emotional conditions of patients
Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
Foster positive relationships and companionship with patients by conversing or reading aloud to help keep them mentally healthy and alert
Other duties as assigned Qualifications:
High School Diploma or GED
Graduated from an accredited Home Health Aid program
One year of previous experience as a Direct caregiver, Home Health Aide, or similar role is preferred
First aid and CPR certification required
Experience in administering prescribed medications and treatments
Extensive knowledge of best practices in the provision of direct care
Ability to adhere to all health and safety guidelines
Excellent communication and interpersonal skills
Ability to handle physical aspects of the job, including bending, stooping, lifting, pushing, pulling, reaching, and walking for periods of time
Ability to perform basic housekeeping duties, run errands, prepare meals, and safely discard biological waste
Willingness to work irregular hours, including evening hours, weekends, and holidays
CNA certification preferred
