Job Description
Overview We are seeking a compassionate and dedicated Direct Support Professional to join our team. In this role, you will provide essential support to individuals with varying needs, helping them achieve their personal goals and maintain a high quality of life. This position is ideal for those with experience with individuals with mental disabilities and who are passionate about caregiving. Duties Demonstrates knowledge and abide by all established policies and procedures of the agency, county, and state. Demonstrates exemplary leadership skills during day-to-day operations. Provide/assure supervision, guidance, and training by the needs and goals of the people/person you are supporting. Transport individuals when needed and provide hand over and hand support in the community. Provide/assure continuity and comfort in daily routine. Assure that the person's goals and behavior programs are implemented as written. Attend all required meetings about the people/person you support. Record and report all required data to Program Specialist/Lifesharing Specialist/Supported Living Specialist. Demonstrates the ability to take appropriate steps in the resolution, recording, and reporting of emergencies/incidents. Be a positive role model. Prepare and sign the person you are supporting daily logs and ensure all entries are clear and concise. Communicate in the staff log as needed. Report and file incident reports within 24 hours. Report all health concerns immediately to the appropriate person (i.e. doctor, dentist, therapist, and/or supervisor). Document your contacts. Follow prescribed recommended treatments and document them.
Administer medications as prescribed:
observe any self-medicating person. Prepare medication log. Observe, report, and record any adverse effects of the medication. Record all administrations. Observe, report, and record any individual injuries: note the location, nature, extent, and reasons for the injury when known. Observe, report, and record any seizure activity in the log and on appropriate forms. Observe and report to the Specialist and appropriate medical personnel any recurring issues that may affect the person you are supporting well-being. Report medical incidents requiring treatment to the Specialist immediately or to the on-call worker if after hours. Record on the State Incident Report Form. Requirements Must be at least 18 years of age. Must be a high school graduate or have a GED Plus. Must have a valid driver's license Must have a good driving record. Must have a vehicle to use. Strong communication skills and the ability to build rapport with clients. Ability to follow care plans accurately while being attentive to the individual needs of clients. A background check is required prior to employment. Compassionate nature with a genuine desire to help others improve their lives.
