Job Type:
Full-Time (Only apply if you have prior non-medical home care industry experience. Candidates must also successfully complete background and reference checks.) Are you passionate about helping seniors and building relationships in your community? Join our growing team as a Client Care Coordinator and help ensure our clients receive exceptional care while expanding our community presence.
Top Responsibilities Client Care & Quality Assurance:
Conduct client intakes, care assessments, supervisory visits, and regular follow-ups to ensure high-quality care and client satisfaction.
Community Outreach & Referral Development:
Visit hospitals, rehabilitation centers, physician offices, senior communities, and other community organizations to build referral partnerships. Earn additional incentives for generating new client referrals.
Caregiver Recruitment & Care Coordination:
Help recruit caregivers through nursing schools, community organizations, and local events, and coordinate caregiver placements based on client needs. Additional Responsibilities Manage client caseloads and maintain ongoing communication with clients, families, caregivers, and referral partners. Address client concerns, caregiver call-offs, and service issues promptly. Ensure documentation, care plans, and compliance records remain accurate. Qualifications 2+ years of experience in home care, healthcare, case management, client services, or community outreach. Strong communication, customer service, and relationship-building skills. Organized, self-motivated, and able to manage multiple priorities. Experience with HHAeXchange, AxisCare, EVV, or similar softwares preferred Valid driver's license, reliable transportation, and willingness to travel throughout the service area. Why Join Us?
