This role offers an energizing opportunity to provide personalized care, promote independence, and foster a safe, comfortable environment for clients across various care settings. Whether assisting with personal needs or supporting residents in community or home environments, your proactive approach will help enhance quality of life and well-being. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and manual handling techniques such as Hoyer lifts. Monitor patients' vital signs and observe changes in health status, documenting findings accurately to inform care plans. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary, ensuring safety during meal times. Support clients with medication reminders and basic medication administration in compliance with HIPAA regulations and care plans. Provide transportation assistance for appointments, errands, or community outings while ensuring client safety at all times. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Engage in patient observation to identify behavioral or health changes, especially for individuals with dementia, Alzheimer's, autism, or developmental disabilities. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to support individuals with dementia, Alzheimer's disease, neurodevelopmental disorders, or intellectual disabilities through specialized memory care and dementia care techniques. Knowledge of patient monitoring techniques including vital signs measurement and patient transfer methods such as Hoyer lifts and patient personal care assistance. Experience working with diverse populations including geriatrics, individuals with disabilities, and those requiring hospice or palliative care. Familiarity with creating and following care plans while maintaining confidentiality under HIPAA standards. Certification in CPR and First Aid is required; additional certifications such as heavy lifting training or manual handling are advantageous. Strong communication skills for effective client interaction and collaboration with healthcare teams to ensure comprehensive support.
