Jenkintown, PA (On the road / Field-based) Position Summary We are seeking a dedicated, detail-oriented Home Care Coordinator to manage client onboarding and ongoing field quality care initiatives for our home care agency. This role is strictly operational and care-focused, with zero sales or marketing requirements. You will serve as the essential field presence for our clients in the Jenkintown area—guiding families through the initial intake process, conducting scheduled compliance check-ins, and executing unannounced pop-up visits to ensure our caregivers maintain the highest standards of safety and service.
Key Responsibilities Intake & Onboarding Coordination Lead Management:
Respond promptly to incoming inquiries, phone calls, and web leads from families and referral networks looking to establish home care services.
Assessments & Discovery:
Conduct initial field consultations with prospective clients and families to evaluate care needs, outline service offerings, and establish proper care expectations.
Care Integration:
Facilitate a seamless transition from the initial inquiry to the formal care kickoff, collaborating closely with internal care coordination and clinical teams.
Documentation & CRM:
Maintain precise, real-time intake statuses and client records in our scheduling and tracking systems. Field Operations & Quality Assurance Visits 90-
Day Quality Reviews:
Conduct scheduled, comprehensive 90-day in-home check-ins with existing clients to review care plans, evaluate client satisfaction, and address any evolving care needs.
Pop-Up Visits:
Perform routine, unannounced pop-up visits at active client residences to observe field caregivers in action, ensuring company policies, safety protocols, and plan-of-care tasks are actively being followed.
Ad-Hoc & Urgency Visits:
Drop in for situational visits as needed—including new caregiver introductions, post-incident investigations, or emergency field troubleshooting to resolve client friction immediately.
Caregiver Onboarding Support:
Assist with the practical field training and initial onsite orientation of new caregivers to ensure they are comfortably acclimated to the client's environment.
Qualifications Experience:
Minimum of 1-2 years of experience in home care intake, healthcare coordination, scheduling, or a hands-on field operations role within the senior care industry.
Local Knowledge:
Strong familiarity with the Jenkintown, PA community and surrounding Montgomery County areas.
Communication Skills:
Outstanding interpersonal and empathetic communication skills; absolute comfort addressing client concerns and managing sensitive care transitions face-to-face. Aptitude for the

A reliable self-starter who excels working independently on the road, balancing a fluid daily schedule of scheduled appointments and surprise visits.
Transportation:
Reliable transportation, active auto insurance, and a valid driver's license for continuous local travel to client homes. Must be able to speak Spanish and English.
