Job Description

Horizons Personal Care, located at the Villa Crest, is recruiting for Personal Care Aides, who meet the following requirements:
PURPOSE:
To provide day to day resident care. Meet the overall resident service needs as outlined in the support plan. Duties include some light housekeeping and activities. Upon training, will be responsible for passing medication and documenting medication administration.
AGE/EDUCATION
  • Must be 18 years of age or older
  • High school diploma or
GED EXPERIENCE
  • Training available for right candidate, otherwise 1-2 years as personal care aide
CERTIFICATION
  • May be a certified nursing assistant
  • C.P.R. /First aide
  • D.
P.W. competency test
QUALIFICATIONS
Must be:
  • Able to work 8 hour shifts, may be called upon to cover for call offs
  • Free of medical conditions including substance abuse
  • Able to communicate with older adult residents
  • Take vital signs including temperature, BP, pulse
  • Recognize changes in resident conditions and report them to Supervisor
  • Push/pull objects weighing up to 50 pounds
  • Able to learn and use EMR (PCC) documentation system
ESSENTIAL FUNCTIONS
  • Assists residents with activities of daily living and independent activities of daily living
  • Takes routine vital signs such as BP, temperature, pulse
  • Reads and understands resident support plans
  • Reports any unusual medical condition change to Supervisor
  • Follows all infection control policies/procedures including covid protocols
  • Assists, as necessary, with non-nursing tasks such as light housekeeping, activities, dietary
  • Records all resident care into PCC
  • Assists with staff orientation of new employees as necessary
  • Provides medication administration services following training by a graduate of the "Trainer Course
  • Upon training, medication administration services provided by the aide will be documented according to the policy
  • Attends all mandatory in-services
  • Performs other duties as assigned
