This vital role involves providing personalized care in clients' homes, ensuring their safety, comfort, and independence. You will be a trusted partner in helping clients maintain their quality of life through attentive caregiving, companionship, and health monitoring. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, and temperature to track health status Administer medications accurately following prescribed care plans while maintaining HIPAA compliance for confidentiality Support clients with meal preparation, ensuring nutritious and balanced diets tailored to individual needs Employ proper techniques such as Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges Maintain a clean and safe environment through light housekeeping tasks like cleaning and laundry Observe and report any changes in client condition or behavior to healthcare professionals promptly Skills Proven experience in home & community care or assisted living environments Knowledge of patient observation, dementia care, Alzheimer's care, and memory care techniques Ability to work effectively with individuals with developmental disabilities or autism spectrum disorder Strong understanding of medication administration, care plans, and patient monitoring procedures CPR and First Aid certification to respond swiftly during emergencies Experience working with residents in nursing homes or group homes is highly desirable Excellent communication skills for collaborating with families, healthcare teams, and social workers Physical ability for heavy lifting and assisting with mobility needs; familiarity with Hoyer lifts preferred Compassionate attitude with a focus on patient-centered support and dignity preservation Embark on a rewarding career where your skills directly enhance the well-being of those you serve.
