Your efforts will help clients maintain independence, improve their quality of life, and feel supported every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs and reporting any changes promptly Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Administer medication following care plans and ensure proper documentation in compliance with HIPAA regulations Support clients with memory care needs such as Alzheimer's or dementia, creating a safe and calming environment Help with light housekeeping tasks like cleaning, laundry, and maintaining a tidy living space Use specialized equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges Collaborate with healthcare professionals to develop and follow personalized care plans Provide emotional support and companionship to promote mental well-being Follow all safety protocols including CPR and first aid procedures to ensure client safety at all times Requirements Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred Knowledge of patient observation, medication administration, and vital signs monitoring Certification in CPR and first aid is required; additional certifications such as HIPAA training are a plus Experience working with individuals with developmental disabilities, autism, or memory care needs is advantageous Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts Strong communication skills and compassionate attitude towards clients of all ages and backgrounds Ability to work independently while following detailed care plans accurately Valid driver's license and reliable transportation are preferred for community-based visits Join us today to become part of a team that values your dedication!
