Job Description
Job Summary We are seeking a dynamic and compassionate Health Home Care Manager to lead patient-centered care coordination within our integrated health services. In this vital role, you will oversee comprehensive case management for individuals with complex health needs, ensuring seamless access to medical, behavioral health, social services, and community resources. Your energetic approach will empower patients to achieve optimal health outcomes while fostering collaboration among multidisciplinary teams. Duties Conduct thorough patient assessments, including medical history review, physiological evaluation, and psychosocial screening to develop individualized care plans. Coordinate inpatient, outpatient, and post-acute care services across hospital settings such as Level I Trauma Centers, Level II trauma centers, PICUs (Pediatric Intensive Care Units), and emergency departments. Facilitate discharge planning by collaborating with healthcare providers, families, and community resources to ensure smooth transitions of care. Manage electronic health records (EHR) and medical records in compliance with HIPAA regulations while documenting all patient interactions accurately. Provide clinical case management for patients with complex conditions including mental health disorders, substance use disorders, disabilities, and chronic illnesses. Engage in diagnostic evaluations and behavioral health assessments to inform treatment strategies. Work closely with multidisciplinary teams to prioritize tasks effectively, ensuring timely interventions and optimal patient service delivery. Support patients' families through education, counseling, and coordination of community resources tailored to diverse cultural backgrounds. Skills Extensive hospital experience including inpatient care at Level I Trauma Centers and ICU settings. Proven expertise in acute care environments such as emergency medicine and hospital medicine. Strong background in pediatric care (PICU), behavioral health, addiction counseling, and mental health counseling. Familiarity with managed care principles, utilization management, discharge planning, and post-acute care coordination. Proficiency in electronic health records (EHR) systems and medical terminology; knowledge of HIPAA compliance. Excellent patient assessment skills combined with physiology knowledge to inform clinical decision-making. Ability to work effectively with individuals from diverse cultural backgrounds and those with disabilities or substance use disorders. Skilled in client communication, working with families, task prioritization, and community resource coordination within social work frameworks. Experience working in outpatient clinics or community-based settings; inpatient experience is highly valued.
