Job Summary We are seeking an energetic and organized Home Care Coordinator to join our dedicated healthcare team. In this vital role, you will serve as the primary point of contact for patients, caregivers, and healthcare providers, ensuring seamless coordination of home care services. Your proactive approach will help deliver personalized, efficient, and compassionate care to clients in their homes. Duties Coordinate and schedule home care services by liaising with patients, caregivers, and healthcare professionals to meet individual needs. Manage patient records using Electronic Health Records (EHR) and Patient Management Software, ensuring accuracy and confidentiality at all times. Communicate effectively via phone and client communication channels to answer inquiries, provide updates, and resolve issues promptly. Support medical administrative tasks such as data entry, appointment scheduling, and maintaining patient confidentiality in compliance with health regulation policies. Collaborate with medical staff to ensure proper documentation, adherence to health regulations, and smooth delivery of care services. Assist with insurance verification processes and understanding health insurance policies relevant to patient care. Maintain a thorough understanding of medical terminology and health regulation policies to support accurate documentation and compliance. Qualifications Proven experience working in a medical office or healthcare setting, with familiarity in medical administrative support roles. Proficiency in Microsoft Office Suite (Word, Excel, Outlook) for documentation and communication purposes. Knowledge of health regulation policies related to patient privacy and confidentiality (e.g., HIPAA). Experience with Medical Office Software such as EMR/EHR systems or Patient Management Software is highly preferred. Strong customer service skills with the ability to communicate clearly via phone and in person. Understanding of health insurance processes and billing procedures. Excellent organizational skills with attention to detail; typing skills for accurate data entry are essential.
