Job Description

The Care Coordinator will be responsible for ensuring client care is delivered correctly as well as ensuring all required paperwork is accurate and complete. This individual will play a crucial role in building and maintaining long-term relationships with Calico Home Care's patients/families and employees. The Relationship Marketing Manager should have a strong understanding of Home Care and Home Health, customer relationship management (CRM) practices, marketing strategies, and be skilled at project management and team leadership. Responsibilities Promptly follow up on client/patient leads. Work with the families to create appropriate Plan of Care. Coordinate the start of care with appropriate team members (including Director of Nursing for IHSS/skilled patients) Complete in person intake and ongoing supervisory visits timely. Manage EMR systems and databases. Collaborate with case management agencies to ensure correct plan of care and PARs are received timely. Some HR duties including placing ads, conducting interviews, background checks, scheduling, supervisory visits, etc. Creating and upkeep of the schedule for both Caregivers and clients. Plan and manage events aimed at increasing public awareness. Cultivate community relationships. Attend events and promote Calico Home Care in the community. Coordinate with management team to generate leads and close deals. Reengage aged leads. Obtain and retain relevant certifications. Fill in for last minute call offs. Other duties as needed. Qualifications Proven experience as a Care Coordinator or similar role. Bilingual (Spanish/English) is preferred, but not required. Knowledge of Home Care/Home Health is a plus. Experience in sales and customer service is a plus. Proficient in MS Office and CRM/project management software. Understanding of customer relationship management (CRM) practices. Excellent communication and interpersonal skills. Proven ability to manage a team. Strong organizational skills. Innovative and unconventional thinking. Understanding of industry specific regulations. Understanding of Agency's policies and procedures. Understanding of Medicaid waiver programs. Care Coordinator Reporting Structure:
Reports to the Owner/Administrator.
Physical Requirements:
o Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with hands and arms. o Ability to frequently use hands to finger, handle, or feel. o Ability to occasionally climb stairs. o Ability to use all senses including sight, hearing, touch, smell, taste. o Ability to frequently lift and/or move up to 15 pounds and occasionally lift and/or move 100 pounds. o Ability to perform the anticipated level of physical assist required to provide safe client care. If the anticipated level of assist is greater than that which the employee can safely provide, the supervisor is to be notified immediately. o Ability to use close vision, distance vision, color vision, peripheral vision, depth perception. o Ability to adjust focus.
Work Environment:
Office setting with some remote work flexibility. Travel will be required for client meetings or industry events. Frequent exposure to outside weather conditions, wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals. Work environment is highly variable, as care is provided in private homes. Lighting, acoustics, air quality, temperature and environmental hazards are not predictable. The noise level in the office work environment is usually moderate. Travel Frequent travel is required.
Salary:
Salary is based upon candidate experience and qualifications, as well as market and business considerations.
Pay Range:
$23-$25/hr. Mileage reimbursement at the federal rate.
Location:
Office location in Greenwood Village. Travel within the eleven counties covered by Calico Home Care's licensing.
