We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. Our mission : Through the talents of an experienced and diverse workforce, OCES supports the independence and dignity of older adults and individuals with disabilities by providing essential information and services that promote healthy, safe living which positively impacts our community. We are seeking a Home Care Program Care Manager to work in our Home Care department to plan, organize, manage and evaluate HC Program operations and assign care management staff. What you'll do: Provide oversight and direction to assigned staff Oversee all care management aspects of HC program Contribute to the development and implementation of procedures, standards and guidelines for program/department operations Ensure program integrity and regulatory compliance Implement quality measures' conduct quality reviews to ensure they are met Participate in planning and execution of comprehensive staff development and training programs Participate in program analysis and evaluation including determination of service needs/gaps and the quality and effectiveness of services delivered Advise and consult with other HC management staff from various disciplines on a regular basis As directed, represent OCES at external meetings and participate in public outreach/education activities and events Maintain a detailed knowledge of current OCES policies, and standards, funding source regulations Maintain accurate documentation What you'll bring: Your passion for helping others and desire to make a difference. Adaptability to be able to change as the needs of consumers or/ programs change Minimum of five years of experience in the human services field; including two years in a supervisory capacity ASAP experience preferred, experience in organization, administration and supervision Excellent written and verbal skills.
