We specialize in companion and personal care services that support our clients' independence and dignity in the comfort of their own homes. About This Role The Lead CNA is the cornerstone of our field care team. This is not a traditional CNA position — it is a leadership opportunity for an experienced caregiver who is ready to take on greater responsibility and grow with an agency that values your expertise. As our first Lead CNA, you will serve as the standard-setter for care quality, the primary resource for our Personal Care Assistants in the field, and a trusted partner to our Director of Clinical Services and agency owner. You will launch care relationships with new clients, guide less experienced caregivers, and play a central role in building a team and culture we are all proud of. This role starts part-time, but for the right person. It is a foundation — not a ceiling. Key Responsibilities Client Care Launch Conduct the first 2-3 visits with each new client to implement the individualized care plan, establish routines, and set the standard for care delivery Observe and assess client needs during the onboarding period and communicate findings to the Director of Clinical Services Assist with the transition of a client's care services to newly assigned caregiver to ensure that the care plan is carefully followed Provide direct personal care services including bathing, grooming, oral hygiene, dressing, toileting, skin care, and mobility assistance Assist with ambulation, range of motion exercises, and safe transfers per the care plan Prepare nutritious meals and assist with feeding when necessary; perform light housekeeping as appropriate PCA Mentorship & Field Support Orient incoming Personal Care Assistants to individual clients, care plans, documentation requirements, and agency expectations Serve as a field resource for PCAs with questions or concerns between scheduled supervisory check-ins Model professional conduct, compassionate care, and thorough documentation for less experienced team members Shift Coverage Serve as a reliable first resource for filling vacant shifts when a PCA is unavailable Maintain schedule flexibility to respond to coverage needs across the Durham and Chapel Hill service area Communicate proactively with the scheduling team (Director of Clinical Services and agency owner) regarding availability and any field concerns Documentation & Communication Document all care provided accurately and promptly using the agency's scheduling and EVV system (WellSky) Adhere to all agency policies, documentation standards, and NC DHSR requirements
Qualifications Required:
Current, active CNA certification in the state of North Carolina Minimum 2 years of experience in personal care, home health, or a related caregiving setting At least 18 years of age Current CPR/First Aid certification (or willingness to obtain prior to start date) Negative TB skin test or CXR per NC DHSR requirements Ability to pass criminal background check per NC G.S. 131E-265 and NC Health Care Personnel Registry clearance Reliable transportation, valid NC driver's license, and personal vehicle insured per NC requirements Physical ability to perform caregiving tasks including standing, bending, lifting (up to 50 lbs with assistance), and assisting with client transfers
Preferred:
Experience working with clients with Parkinson's disease, PSP, dementia, ALS, or other neurological or movement disorders Prior experience in a lead, senior, or mentorship role in a caregiving setting Experience with home care or community-based settings (vs. facility-only) Familiarity with electronic visit verification (EVV) systems; WellSky experience a plus

Specifically, the Lead CNA will not administer medications, take or transcribe physician's orders, or perform procedures requiring nursing licensure or sterile technique. Schedule, Compensation & Growth This position begins as part-time with a flexible, variable schedule. Schedule flexibility — including availability for early morning, evening, and weekend shifts and willingness to fill coverage gaps — is essential to this role. As our client roster grows, so will the hours and scope of this position. Compensation is competitive and commensurate with experience; a shift differential reflecting the lead responsibilities is included.
