Job Description
Job Overview We are seeking a compassionate and dedicated Personal Care Assistant to join our team in providing exceptional support to individuals with diverse needs. In this vital role, you will assist clients with daily activities, promote independence, and ensure their safety and well-being. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using proper manual handling and patient transfer methods such as Hoyer lifts when necessary Monitor patients' vital signs, observe for changes in condition, and report findings accurately to healthcare professionals Support medication administration following HIPAA guidelines and care plans to ensure safety and compliance Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Provide personal transportation assistance for appointments, errands, or community outings to promote independence and social engagement Maintain a clean, safe environment by performing light cleaning duties and ensuring infection control standards are met Document patient observations, care provided, and any incidents clearly and thoroughly in accordance with home health protocols Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of dementia care, Alzheimer's care, memory care, hospice care, or working with individuals with neurodevelopmental or intellectual disabilities is preferred Ability to perform patient monitoring tasks such as vital signs measurement and patient observation with attention to detail Familiarity with care plans, medication administration procedures, CPR/First Aid certification, and HIPAA regulations is essential Experience working with individuals with autism or developmental disabilities is a plus Physical ability to lift heavy objects safely using proper manual handling techniques and assist with patient transfers using equipment like Hoyer lifts Strong communication skills to effectively interact with clients, families, and healthcare team members Compassionate attitude combined with a proactive approach to caregiving ensures high-quality support for residents' physical and emotional needs Join us in delivering compassionate care that transforms lives!
