Duties Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by checking vital signs and observing for any changes in condition Prepare nutritious meals tailored to dietary requirements and assist with feeding as needed Administer medications accurately following care plans and HIPAA privacy regulations Support patients with cognitive challenges by engaging in memory care activities and providing companionship Operate mobility aids such as Hoyer lifts or wheelchairs to ensure safe transfers and movement Maintain detailed documentation of care provided, patient observations, and any incidents or concerns Perform light housekeeping tasks including cleaning, laundry, and ensuring a safe environment for residents or clients Follow established care plans created by healthcare professionals or social workers to meet individual needs Respond promptly to emergencies by executing first aid procedures or contacting emergency services Requirements Proven experience in home health, assisted living, nursing home environments, or group homes Knowledge of patient observation techniques and vital signs monitoring Certification in CPR and First Aid is required; additional certifications such as HIPAA compliance are preferred Experience working with individuals with developmental disabilities, autism, Alzheimer's disease or dementia is highly desirable Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina for long shifts is essential Strong communication skills for effective interaction with patients, families, and healthcare teams Compassionate attitude with a genuine desire to improve the quality of life for others Flexibility to work various shifts including evenings, weekends, or holidays as needed Join our team to deliver heartfelt care that empowers individuals to live their best lives!
