Job Description

Join Our Team as a Relative Caregiver Care Coordinator! Join our mission to provide essential support and coordination for relative caregivers and the youth they care for. We're looking for a compassionate and dedicated Relative Caregiver Care Coordinator to join our team. In this role, you'll be a vital resource for families, helping them navigate complex systems, access services, and build a strong support network.
What You'll Do:
As a Care Coordinator, you will be the main point of contact for the families you serve. You'll use your skills to:
Provide Case Management:
You'll coordinate all aspects of a family's care, from attending court hearings and team meetings to conducting home visits and developing comprehensive service plans. You'll connect families with community resources, ensuring they have the support they need to thrive.
Coach and Empower Families:
You'll teach positive parenting and problem-solving skills, empowering caregivers to feel confident and self-sufficient. This includes providing parent training and crisis intervention when needed.
Build Relationships:
You'll develop and maintain professional relationships with various community agencies to ensure your families get the best possible care. You'll also participate in workshops and conferences to enhance your own professional growth.
Handle Logistics:
You'll be responsible for timely and accurate documentation and may need to transport children and families to appointments.
What We're Looking For:
Education:
A bachelor's degree in social work or a related field.
Experience:
At least one year of experience working with youth and families.
Requirements:
