In this role, you will provide personalized care in clients' homes, promoting independence, safety, and comfort. Your proactive approach will help foster a positive environment for those you serve, ensuring their needs are met with professionalism and kindness. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene Support clients with mobility and transfers using manual handling techniques and equipment such as Hoyer lifts and patient transfer methods Prepare meals tailored to client wants, dietary needs and assist with meal consumption when necessary Monitor clients for any changes in health or behavior, reporting findings accurately in care plans Provide companionship and emotional support Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces Support clients during community outings or personal transportation needs using appropriate safety measures Communicate clearly with clients' families and healthcare teams to coordinate care plans and report any concerns or observations Skills Proven experience working in home health, assisted living facilities, nursing homes, or group homes with a focus on geriatrics or disability support Strong patient observation skills to detect changes in condition or behavior promptly Ability to assist with ADLs using proper manual handling techniques and patient transfer methods Excellent client communication skills with a compassionate approach to caregiving Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information Capable of pivot transfers and use of gait belt when required for patient transfers or mobility assistance Ability to work independently while following detailed care plans and resident rights guidelines This role is ideal for motivated caregivers eager to provide high-quality home-based support that enhances quality of life.
