This energetic and dedicated position offers the opportunity to provide personalized care in clients' homes, ensuring safety, comfort, and independence. You will be a trusted support for seniors, individuals with disabilities, and those recovering from illness, delivering care that promotes dignity and well-being every day. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' vital signs and observe changes in health status to report to healthcare professionals Prepare nutritious meals tailored to dietary needs and assist with medication administration following care plans Operate mobility aids such as Hoyer lifts and ensure safe transfer techniques to prevent injury Maintain a clean and safe environment through regular cleaning, laundry, and organization of living spaces Document patient observations, care provided, and any incidents accurately in compliance with HIPAA regulations Support clients with cognitive impairments such as Alzheimer's disease or dementia through memory care techniques and engagement activities Experience Prior experience in home health, assisted living, nursing homes, or group homes preferred Demonstrated ability to work with individuals with developmental disabilities or autism spectrum disorder Knowledge of patient monitoring, vital signs collection, and basic nursing skills such as CPR and first aid certification required Experience working with residents who have dementia or require hospice or hospice-like care is a plus Familiarity with creating and following personalized care plans aligned with healthcare standards Ability to perform heavy lifting safely and operate medical equipment like Hoyer lifts effectively Strong communication skills for working collaboratively with families, healthcare teams, and social workers Join us to make a meaningful difference in people's lives by providing compassionate, professional home care.
