Home Care Outreach & Sales Specialist CareChoice Home Care Dallas-Fort Worth, TX Full-time | Field + Office About the Role CareChoice Home Care is seeking an entrepreneurial, motivated, and results-driven Home Care Outreach & Sales Specialist to help grow our Medicaid waiver consumer base throughout the Dallas-Fort Worth market . This is primarily a hunting and business development position. Your primary responsibility will be to go into the community, build relationships, develop referral sources, generate new opportunities, and ultimately grow the number of Medicaid waiver consumers served by CareChoice . This is an ideal role for someone who enjoys networking, sales, community outreach, field marketing, relationship building, and creating opportunities from the ground up. You will be expected to treat your territory like your own business—identifying opportunities, building a referral network, getting out into the community, and finding creative ways to drive growth. The role also includes managing the intake and conversion process for the opportunities you generate , ensuring prospective consumers and families have a smooth experience from initial contact through enrollment.
What You'll Do Field Marketing & Community Engagement:
Serve as a visible ambassador for the CareChoice brand throughout the local community. Attend networking events, healthcare events, senior expos, community events, job fairs, conferences, and other opportunities to promote CareChoice. Identify organizations and community events where CareChoice should have a presence. Build relationships with nonprofits, senior centers, community centers, faith-based organizations, healthcare organizations, and other local partners. Develop grassroots marketing and outreach opportunities that generate awareness and new business. Work with the marketing team on local campaigns, events, partnerships, and community initiatives. Identify local market trends, competitor activity, and opportunities for growth.
Intake & Consumer Conversion:
Manage the intake process for qualified opportunities generated through your outreach and business development efforts. Serve as a knowledgeable and professional point of contact for prospective consumers and families. Explain CareChoice services and Medicaid waiver programs. Verify eligibility and coordinate required documentation. Follow up with prospective consumers to maximize conversion. Coordinate with internal teams to ensure a smooth transition into services. Maintain accurate lead, referral, and consumer information in the CRM. We're Looking For Someone Who... Has an entrepreneurial mindset and takes ownership of their results. Can work independently and manage their own territory and schedule. Is creative and resourceful when developing new business. Builds genuine relationships and understands that strong relationships lead to referrals. Is comfortable with rejection and willing to keep pursuing opportunities. Has excellent communication, presentation, organization, and follow-up skills.
Qualifications:
Previous experience in sales, business development, field marketing, healthcare sales, home care, community outreach, or referral development preferred. Experience generating new business or developing referral relationships strongly preferred. Healthcare, home care, Medicaid, or Medicaid waiver experience is a plus. Strong networking and relationship-building skills. Comfortable with face-to-face sales and community outreach. Ability to manage a pipeline and work toward measurable goals. Experience with CRM systems and Microsoft Office. Bilingual English/Spanish strongly preferred. Reliable transportation and ability to travel throughout the Dallas-Fort Worth area required.
Travel & Training Requirements:
This is a field-based role , and regular travel throughout the Dallas-Fort Worth market is required. The ability to commute between Dallas and Fort Worth and surrounding communities is essential. The selected candidate will also be required to complete one week of training at CareChoice Home Care's headquarters in Pennsylvania .
How Success Is Measured:
The primary measure of success in this position is growth in Medicaid waiver consumers. You will be expected to build a strong referral network, generate qualified opportunities, convert those opportunities into consumers, and contribute directly to the growth of the Dallas-Fort Worth market.
