PERSONAL SUPPORT ASSISTANT

(Residential Care Asst./Respite Care Asst.)

QUALIFICATIONS

1. High School Diploma or its equivalent. 2. Be at least 18 years of age. 3. Possession of a valid driver's license for the State of Texas if providing transportation.

RESPONSIBILITIES

Residential Assistance:

1. Direct personal assistance with activities of daily living (grooming, eating, bathing, dressing, and personal hygiene). 2. Assistance with meal planning and preparation. 3. Securing and providing transportation in accordance with state law. 4. Assistance with housekeeping. 5. Assistance with ambulation and mobility. 6. Reinforcement of counseling and therapy activities. 7. Assistance with medications and the performance of tasks delegated by a Registered Nurse. 8. Supervision of individual's safety and security. 9. Facilitating inclusion in community activities, use of natural supports, social interactions, participation in leisure activities, and development of socially valued behaviors; and 10. Habilitation, exclusive of Day Habilitation. 11. All other services in accordance with principles set forth in the specific waiver program.

Respite Care:

1. Assisting with training in self-help and independent living skills. 2. Provision of room and board when provided overnight in a setting other than the person's normal residence. 3. Support for all consumers of Home and Community Based Services in need of emergency or planned short-term care. 4. Be provided on a 24-hour increment or any part of that increment for no more than 30 days respite per year per consumer. 5. Assistance with on-going provision of needed waiver services; and 6. Assistance with securing and providing transportation. All other services in accordance with principles set forth in the specific waiver program.
