Job Description

Job Overview We are seeking a compassionate and dedicated Caregiver to assist a resident with attending appointments at their designated address. This vital role involves providing personalized support to ensure the individual's safety, comfort, and independence during travel and appointments. The ideal candidate will be proactive, attentive, and experienced in delivering high-quality care within a home or community setting. This paid position offers an opportunity to make a meaningful difference in someone's daily life while working in a supportive environment. Responsibilities Accompany the resident to medical, dental, therapy, or other scheduled appointments, ensuring safe transportation and punctuality Assist with mobility needs using appropriate equipment such as Hoyer lifts or wheelchairs when necessary Support the resident with activities of daily living (ADLs), including dressing, grooming, and personal hygiene during outings Observe and monitor the resident's health status, noting any changes or concerns related to vital signs, medication adherence, or overall well-being Ensure the environment remains clean and safe before, during, and after appointments by performing light cleaning tasks as needed Follow care plans meticulously, including medication administration and documentation in compliance with HIPAA regulations Provide emotional support and companionship to promote a positive experience during outings Requirements Proven experience in home & community care, assisted living, nursing homes, or group home settings Knowledge of patient observation techniques and ability to monitor vital signs accurately Certification in CPR and First Aid is required; additional training in Alzheimer's care, dementia care, memory care, or hospice care is a plus Familiarity with working with individuals with developmental disabilities, autism spectrum disorder, or other special needs Ability to perform heavy lifting and assist with mobility aids safely while adhering to safety protocols Excellent communication skills to coordinate effectively with healthcare professionals and family members Valid driver's license and reliable transportation for appointment travel Join us in providing compassionate support that empowers individuals to maintain their independence and quality of life. $16.00 - $18.00 per hour
