Responsibilities Provide direct personal care assistance to residents, including help with bathing, grooming, dressing, and mobility. Assist with Activities of Daily Living (ADLs) such as eating, toileting, and transferring using proper manual handling techniques and patient transfer methods like Hoyer lifts. Monitor and observe patients' conditions regularly, recording vital signs and reporting any changes to healthcare professionals promptly. Support medication administration following prescribed care plans while adhering to HIPAA regulations to ensure patient privacy and confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist residents with meal consumption when necessary. Maintain a clean and safe environment by performing light housekeeping duties including cleaning resident areas and ensuring infection control standards are met. Facilitate transportation for appointments or outings using personal transportation assistance or facility vehicles while ensuring resident safety at all times. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients closely and accurately document health status; experience with dementia care, Alzheimer's care, memory care or neurodevelopmental disorders is advantageous. Knowledge of patient monitoring techniques including vital signs measurement, medication administration, and first aid/CPR certification. Familiarity with resident's rights, HIPAA compliance, and confidentiality standards within healthcare settings. Ability to perform manual handling tasks including heavy lifting and patient transfers safely and effectively. Strong communication skills for engaging with clients and collaborating with healthcare teams; experience working with individuals with disabilities or developmental delays is a plus. Valid driver's license or reliable transportation for assisting with personal transportation needs; willingness to work flexible hours including weekends or evenings as required.
