Full Job Description Job Summary:

Provide personal assistance and/or health-related services at the patient's home

Functions:

Promote positive, supportive, and respectful communication with patient/family Provide an atmosphere of respect for patient's privacy and property Provide individualized tasks based on patient's needs (ex. Bathing, Dressing & Grooming) May provide general light housekeeping activities as required by patient/family Report changes in patient's condition to ensure continuity of care Report unsafe conditions and problems to agency management

Qualifications:

Must be 18 years of age or older Ability to communicate both verbal and written Capable of working responsibly with confidential information Must be able to work independently Must be able to undergo and pass a background check
