Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health by observing vital signs, managing medication administration, and updating care plans accordingly. Support residents with mobility using equipment such as Hoyer lifts and patient transfer methods to ensure safe movement. Prepare nutritious meals tailored to dietary needs and assist with meal consumption as necessary. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Provide companionship and emotional support to foster a positive living environment. Document patient progress accurately while adhering to HIPAA regulations and confidentiality standards. Requirements Proven experience working with individuals with disabilities, developmental or intellectual disabilities, or seniors in settings such as group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient care procedures including medication administration, vital signs monitoring, first aid, CPR certification, and manual handling techniques like heavy lifting and patient transfer methods. Familiarity with dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism. Ability to work independently while maintaining excellent communication with clients' families and healthcare teams. Experience working with diverse populations including geriatrics and individuals with neurodevelopmental or developmental disabilities. Knowledge of HIPAA compliance to ensure privacy and confidentiality of patient information. Valid certifications in first aid and CPR; additional certifications in home health or hospice care are a plus. Physical ability to perform manual handling tasks safely; capable of assisting with ADLs and providing personal transportation assistance when needed.
