In this vital role, you will provide personalized assistance and support to individuals with diverse needs, promoting their independence, safety, and well-being. Your energetic approach will help create a warm, respectful environment where clients feel valued and cared for. This position offers an opportunity to make a meaningful difference in the lives of those you serve through attentive caregiving, health monitoring, and daily assistance. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs such as blood pressure, pulse, and temperature; report any concerns promptly Support medication administration following care plans and HIPAA regulations to ensure patient confidentiality and safety Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Maintain a clean and safe environment through regular cleaning duties and proper infection control practices Observe and document patient conditions, behaviors, and progress to inform care plans and communicate effectively with healthcare teams Requirements Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care settings is highly preferred Knowledge of patient observation techniques, Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or autism Strong communication skills to effectively collaborate with clients, families, and healthcare professionals Experience in medication administration, vital signs monitoring, and developing or following care plans is a plus Demonstrated compassion for seniors or individuals with disabilities; commitment to providing respectful and dignified care Join us in delivering exceptional personal care that transforms lives!
