About the role Visiting Angels of Olympia is looking for a Client Care Coordinator to take ownership of a caseload of home care clients — from the first assessment through ongoing care plan oversight and caregiver supervision. This is a relationship job as much as a compliance job. You'll be the person clients and families call when something needs to change, the person caregivers turn to when they have a question in the field, and the person who makes sure the care we deliver actually matches the plan on paper. Roughly half your day is spent out of the office: client homes, referral partners, community meetings. You'll report directly to the Administrator and serve as a designated Alternate Supervisor of Direct Care Services. What you'll do Client care management Conduct initial in-home client assessments and build individualized Plans of Care with client consent Review and revise Plans of Care every six months, annually, and any time care needs change Make sure services start on time, discharges get proper notice, and we only take on clients we're truly equipped to serve Receive and resolve client concerns and complaints Set up and maintain client files and document changes electronically Caregiver supervision Supervise caregivers assigned to your caseload — performance, competency, and Plan of Care compliance Verify caregiver competency for transfers, mechanical lift operation, medication assistance, and other care-specific tasks Review each Plan of Care with assigned caregivers and document supervisory visits and calls Partner with the Scheduler to match caregivers to clients based on skills and competence Respond to caregiver questions, concerns, and incident reports promptly Compliance and quality Ensure practice stays aligned with WAC 246-335 and agency policy Contribute to policies and procedures for admission, transfer, discharge, and referral Participate in monthly quality improvement meetings Community relations Build and maintain relationships with referral sources and community partners Attend community support groups and support philanthropic activities Other Share compensated after-hours on-call phone coverage on a rotating basis Provide cross-coverage for the other Client Care Coordinator's caseload as needed Occasional evening or weekend work when operational needs require it What we're looking for Required Degree or certification in nursing, social work, healthcare administration, or a related field — or five or more years of combined education and experience in home care or case management in a healthcare setting Active HCA/CNA License Working knowledge of Washington home care regulations, including

Familiarity with admission, transfer, discharge, and referral best practices in home care Strong communication skills and a real customer service instinct Sharp organizational skills, attention to detail, and comfort with computer systems for scheduling, notes, and records Sound judgment, discretion, and the ability to handle confidential information Genuine interest in working with older adults Valid driver's license and access to a reliable vehicle Ability to pass a background check, SSN verification, and pre-employment drug screen Nice to have Experience writing or implementing policies and procedures under state regulation Experience with recruiting, hiring, and orienting employees Familiarity with medication self-administration assistance, vital signs assistance, passive range of motion exercises, and home care aide practices Involvement with community support groups or philanthropic work in home care Quality improvement experience Visiting Angels is an equal opportunity employer committed to fostering a diverse and inclusive workplace.
